Download the PSA form or the SGC form - Fill it out and then attach the file before submitting.

Slab10s prefer card savers when cards ship.  If you ship without a grading card saver then please state in the PSA or SGC form or message below for permission to move card from top loader to grading card saver sleeve.  Slab10s Trading Cards LLC does not hold any responsibility when transfering from top loader to grading card saver.
